Thank You for choosing Eagle’s View Academy to further your child’s education!

To enroll for the 2020-2021 school year as a New Student you must complete the following:

IMPORTANT: In order to gain admission to Eagle’s View Academy, please collect all of the materials listed and complete the detailed process below. All documents must be submitted together. Once all of the documentation listed below has been received, a representative from the school will contact you to setup an appointment for your student to take the entrance exam. An informational meeting with a school administrator is required prior to enrollment.  The informational meeting can be completed at anytime during the admissions process.

  1. Completed Enrollment Application for Admission.

  2. A copy of the following forms:

    • Transcripts/Report Cards for current and previous school year (Kindergarten applicants excluded).

    • Most recent standardized testing (FSA, SAT, FCAT, etc.).

    • Educational/Psychological Test Results and IEP/504 (if applicable).

    • Scholarship Award Letter (if applicable).

    • Principal/Counselor Recommendation Form – must sent directly from previous school.

    • Request for Student Records Form, completed in full and signed.

    • Copy of Birth Certificate.

    • Florida Immunization Form (Blue 680 Form).

    • Florida School Entry Physical

  3. Complete the Admissions Test/Assessment. (***Students in Grades 1–12)

    • A school representative will contact you to setup an appointment to take the entrance exam.

    • Please bring the $50.00 Testing Fee with you at the time of the test/assessment or make a payment ahead of time with the front office.

 

A school representative will contact you to inform you of your acceptance to Eagle’s View Academy. Upon acceptance, the online Financial Obligation Form will be made available and completion of that form finalizes enrollment.

Guidelines for Admission

Eagle’s View Academy has established a guideline to use for Admissions Standards as listed below.

  1. A minimum of C or above overall average.

  2. Previous standardized testing scores must meet, at minimum, average grade level standards.

  3. A favorable principal recommendation.

  4. Entrance Exam must meet, at minimum, average grade level standards.

Professional Misconduct 

All employees and agents of a public school district, charter school, or private school have an obligation and legal responsibility to report misconduct by instructional personnel and school administrators which affects the health, safety, or welfare of a student. Failure to report misconduct may result in penalties up to termination of employment and revocation of an educator's certificate. Such misconduct may include, but is not limited to, obscene language, drug and alcohol abuse, disparaging comments, prejudice or bigotry, sexual innuendo, cheating or testing violations, physical aggression, and accepting or offering favors.  All incidents of professional misconduct should be reported to:Josh Sheetz,School Administrator, at 904-786-1411 Extension 114

If someone tells you about misconduct, be a LEADER: Listen, Evaluate, Act immediately, Document, Encourage, and Report.

Professional Ethics Standards
In accord with the Florida Ethics in Education Act, all employees of Eagle’s View Academy have a duty to report all suspected or actual cases of child abuse, abandonment, or neglect; have immunity from liability if they report such cases in good faith and have a duty to comply with child protective investigations. There is a legal penalty for not reporting suspected or alleged child abuse or alleged misconduct by instructional personnel or school administrators. The Florida Abuse Hotline is 1.800.962.2873.
