    Admissions Checklist

    IMPORTANT: In order to gain admission to Eagle's View Academy, please collect all of the materials listed and complete the detailed process below. Documents do not need to be sent in a single package. You may send information to EVA as it becomes available. Once all the documents listed below have been received and testing has been completed, you will be notified with the results of the evaluation and an appointment for an interview will be made if applicable.
    1. Completed Enrollment Application for Admission
    2. A copy of the following forms:
    3. Transcript showing courses being taken and grades earned to date and final grades from previous two years (if applicable)
    4. Most recent standardized testing (CTBS, SAT, FCAT, etc.)
    5. Educational/Psychological Test Results and IEP if applicable
    6. Principal/Counselor Recommendation Form – must be mailed directly from previous school
    7. Arrange to take the Admissions Test. (***Students in Grades 1st–12th)
      • Make an appointment with our school office for your student to take the Admissions Test.
      • Please bring the $50.00 Testing Fee with you at the time of the test or make a payment ahead of time to the front office.
    The following documents may be submitted at time of interview or registration:
    • Request for Student Records Form, completed in full and signed
    • Copy of Birth Certificate
    • Florida Immunization Form (Blue 680 Form)
